*These deeds will not remove your name from the mortgage note! The property will need to be refinanced, or an assumption of the mortgage must be worked out with the mortgage company. Otherwise, the property must be sold to remove your name from the note. Be sure to speak with an attorney if you are in this situation!
If you and your spouse own a vehicle and the title needs to be transferred
- Power of Attorney: $200.00
If you or your spouse have a Retirement Plan and the funds needs to be divided
- Qualified Domestic Relations Order is required (third party pricing)
- This does not include fees that the plan may charge for documents or for their lawyers to review the document
- This does not apply to cases where the decree has already been entered

If you or your spouse have an investment account and need to transfer the assets in the account
- Letter of Instructions: $350.00
If Your Spouse Cannot Be Located
In these cases you will be responsible for Filing Fees, Court Costs, and any Ad Litem fees (if your case involves children) that you may incur. Duckworth & Ray, LLP will draft the following documents for an additional charge:
- Attorney's Affidavit of Last Known Address: $350.00
- Client's Affidavit: $350.00
Document Revision Policy
If you wish to have a document revised AFTER it has been drafted, Duckworth & Ray, LLP will make the first revision free of charge. ANY ADDITIONAL REVISIONS WILL BE CHARGED TO YOU BASED ON THE ATTORNEY'S AND STAFF MEMBER'S HOURLY RATE AND ARE NOT INCLUDED IN THE FLAT-RATE PRICE.
